Short Term Medicine Form

SHORT TERM MEDICINE FORM

 

We understand children will need to take medicines during the day at some time during their time with WPS. Usually this will only be for a short period of time, perhaps to finish a course of prescribed antibiotics. 

 

Wherever possible doses should be given before or after school and at bedtime, however medicines may be brought into school if it would be detrimental to the child not to do so. Medicines should be brought to school in the original containers with the labels and any dosage attached.  Please also ensure the box or front of the medicine is clearly labelled with the child’s name and class.

 

Please find below a copy of the Short Term Medicine Form which must be completed in order for the school to administer any medication.  Once completed this form and any medication must be handed to the School Office.
